How does Capomed Lower Blood Pressure?
Capomed is an angiotensin-converting enzyme (ACE) inhibitor used in the treatment of hypertension, congestive heart failure, and various renal syndromes such as diabetic nephropathy and scleroderma.

Capomed has an extremely high affinity for ACE and competes with the natural substrate, angiotemsin I, to block its conversion to angiotensin II. Angiotensin II is a potent vasoconstrictor and a negative feedback mediator for rennin activity, thus, as results of lower angiotensin II plasma levels, blood pressure decreases and plasma renin activity increases.

FORMULATION:
Each Tablet contains:
Captopril .................... 25 mg
Captopril .................... 50 mg

INDICATIONS:
Captopril (Capomed) is used for the management of hypertension, in heart failure, following myocardial infarction and in diabetic nephropathy.

PRECAUTION: 
Elderly patients or patient with peripheral vascular diseases or generalized astherosclerosis may be at high risk because they may have clinically silent disease. Renal function should be checked prior to administration of Captopril (Capomed) and the same should be monitored during treatment. Patients with existing renal disease or those taking large doses of Captopril (Capomed) should be monitored regularly for proteinuria. Regularly white blood cells counts may be necessary in patients with collagen vascular disorders or in those receiving immunosuppressive therapy.

CONTRAINDICATION:
Captopril (Capomed) is not recommended for use in patients hypertensive to it or to other angiotensin-coverting enzyme (ACE) Inhibitors and to those with aortic stenosis or outflow tract obstruction and renovascular disease. The drug is also contraindicated during (especially during the second and third trimesters) and during lactation.

DOSAGE: 
Adult dose to be taken on an empty stomach (1 hour or 2 hours after meals).

For Hypertension:
Initial dose: 12.5 mg twice daily, preferably at bedtime. Increase dosage gradually at intervals of 2 to 4 weeks according to the response.
Initial dose: 6.25 mg twice daily if given in addition to a diuretic or to elderly patients.
Maintenance dose: 25 mg to 50 mg twice daily and should not normally exceed 50 mg three times daily

For heart failure:
Initial dose: 6.25 mg or 12.5 mg thrice daily under medical supervision.
Maintenance dose: 25 mg 2 to 3 times daily and should not normally exceed 50 mg thrice daily.

Following myocardial infarction:
Initial dose (may be started 3 days after myocardial): 6.25 mg once daily, increased over several weeks to 150 mg daily in divided doses if tolerated.

Diabetic nephropathy

For Type 1 diabetics: 
75 mg to 100 mg daily in divided doses.

Special dosage
For patients with severe renal impairment (creatinine clearance of <30mL/min)

Initial dose:
12.5 mg twice daily.Or as prescribed by a physician.

ADVERSE EFFECTS:

The following are some of the reported adverse effects of Captopril (Capomed): nausea, headache, dizziness, taste disturbances, “scalded mouth syndrome”, skin reactions (e.g. Bullous pemphigoid, hyperhidrosis, cutaneous hypersensitivity vasculitis), cough, pneumonitis and cholestatic jaundice.

DRUG INTERACTIONS:
Allopurinol. Fatal Steven-johnson syndrome developed in one patient who received Allopurinol and Captopril (Capomed) concurrently.
Antacids. Administration of Captopril (Capomed) with antacids reduces the bioavailability of Capomed.
Antidiabetics. Hypoglycemia was noted in 3 type 1 diabetics when Captopril (Capomed) was added to their therapeutic regimen.
Azathioprine. Luecopenia has been reported in a patient given Captopril (Capomed) with Azathioprine.
Digoxin. Increased serum-digoxin concentration has been reported in patients with severe chronic failure given Captopril (Capomed)
