How does Buscomed exert its antispasmodic property?

Buscomed relaxes the muscle in the walls of the gastro-intestinal and genito-urinary tracts by blocking the receptors called muscarinic (or cholinergic) receptors that are found on the surface of the muscle cells. This prevents a chemical called acetylcholine from acting on these receptors. Acetylcholine acting on these receptors normally causes the muscle to contract. By reducing this, Buscomed helps the muscle in the GI and GU tracts to relax. This reduces involuntarily contractions and spasms of the muscle.

Buscomed is used to treat painful spasms of the stomach and intestines, reproductive organs and urinary system.

FORMULATION:
1 ampoule of 1 ml contains: 20 mg hyoscine-n-butylbromide
Each tablet contains: 10mg hyoscine-n-butylbromide

MECHANISM OF ACTION:
Hyoscine N-butylbromide is a competitive antagonist of the actions of acetylcholine and other muscarinic agonist. The drug is devoid of central nervous actions. The receptors affected by the drug are those of the peripheral structures that are either stimulated or inhibited by muscarine; such as exocrine glands and smooth and cardiac muscle. Cholinergic nerve stimulations are also inhibited but less readily than are responses to injected choline esters.

Depending on the dose, hyoscine N-butylbromide may inhibit the secretions of the respiratory tract, GIT, and sweat glands; it may reduce the motility and tone in the GIT, ureter, urinary bladder and biliary tract.

INDICATIONS:
Buscomed is indicated in the treatment of spasm and hypermotility of the GIT, biliary and renal colic, and in spasmodic dysmenorrhea. It may also be used in patients with gastric or duodenal ulcer.

DOSAGE and ADMINISTRATION:
Adults and Adolescents over 12 years: 1-2 ampoules of Hyoscine-N-butylbromide (Buscomed 20mg) may be administered by slow intramuscular or intravenous injection several times daily. The maximum daily dose of 100 mg should not be exceeded.

Adults and Adolescents over 12 years: 1-2 tablets of Hyoscine-N-butylbromide (Buscomed 10mg) 3-5 times daily. The maximum daily dose of 100mg should not be exceeded.

 

CONTRAINDICATIONS:
Buscomed should not be given to patients with closed-angle glaucoma or to patient with narrow-angle between the iris and cornea since this may lead to increased intraoccular pressure. It is also contraindicated patients suffering from paralytic ileus. It should be used with caution in patients with prostatic enlargement, coronary insufficiency or cardiac failure. Safe used in pregnancy has not been established.

ADVERSE EFFECTS:
These include dryness of the mouth, thirst, dilation of the pupils with loss of accommodation, photophobia, flushing and dryness of the skin, tachycardia, a desire to urinate with the inability to do so; as well as reduction in the tone and motility of GIT leading to constipation. Vomiting, giddiness and staggering may occur through occurrence is rare.

DRUG INTERACTION / INCOMPATIBILITY:
The effects of Buscomed may be enhanced by the concomittant administration of the other drugs with anticholinergic properties, such as amantadine, some antihistamines, butyrophenones and phenothiazines and tricyclic antidrepressants.
