What is the mode of action of Zolvastin?

“Statin” are the class of drugs that lowers the level of cholesterol in the blood by reducing the production of cholesterol by the liver. Statins block the enzyme in the liver that is responsible for making cholesterol. This enzyme is called hydroxyl-methylglutaryl-coenzyme. A reductase (HMG-CoA reductase for short). Scientifically, statins are called HMG-CoA reductase inhibitors.

Zolvastin is a cholesterol- lowering medicine. It inhibits the production of cholesterol by the liver. It lowers overall blood cholesterol as well as blood LDL cholesterol levels. LDL cholesterol is believed to be the “bad” cholesterol that is primarily responsible for the development of coronary artery disease. Lowering LDL cholesterol levels retards progression and may even reverse coronary artery disease.

FORMULATION:   
Each tablet contains:
Simvastatin .......................... 20 mg
Simvastatin .......................... 40 mg

PHARMACOKINETICS:
Simvastatin (Zolvastin) is readily absorbed from gastro-intestinal tract after oral administration. It is then hydrolyzed to its active form, B-hydroxyacid, and metabolites (active and inactive). Less than 5% of Simvastatin (Zolvastin) goes to circulation as active metabolites, and about 95% of it and its active form are bound to plasma proteins. Primary means of excretion is thru the feces (as metabolites) and about 10-15%is eliminated in the urine as the inactive form.

INDICATION:
Simvastatin (Zolvastin) is used to reduce low-density lipoprotein-cholesterol, apolipoprotein B and triglycerides, and to increase high-density cholesterol in the treatment of hyperlipidaemias. Simvastatin (Zolvastin) is also used as an adjunct treatment in patients with homozygous-familial hypercholesterolaemia, and it is given prophylactically to hypercholesterolaemic patients with ischaemic heart disease. 

PRECAUTION:
Liver function should be assessed prior to initial treatment and should be monitored periodically until one year after the last dosage treatment. Simvastatin (Zolvastatin) must be used with caution in patients with severe renal impairment.

CONTRAINDICATION:
Simvastatin (Zolvastin) should not be given to patient’s hypersensitive to it and to those with acute liver disease or unexplained persistently raised serum-aminotranferase concentrations. Use during pregnancy is not recommended.

DOSAGE:
For patients with ischaemic heart disease
-Initial dose of 20 mg in the evening. Dosage may be adjusted at intervals of not less than 4 weeks up to a maximum of 80mg once in the evening.

For patients with homogynous familial hypercholesterolaemia
-40 mg once in the evening or 80mg daily in three divided doses: 20mg, 20mg and 40mg.

WARNING:
Discontinue therapy if marked or persistent increased in serum-aminotransferase or creatine phosphokinase concentrations occur.

ADVERSE EFFECTS:
The most common adverse effects of Simvastatin (Zolvastin) are gastro-intestinal disturbances. Other includes headache, skin rashes, dizziness, blurred vision, insomnia, and dysgeusia.  Hypersensitivity syndrome (manifested by angioedema), hepatitis and pancreatitis have also been reported, Rhabdomyolysis rarely occur.

DRUG INTERACTIONS:
Ciclosporin, Itraconazole, ketoconazole, Erythromycin, Clarithromycin, HIV-Protease inhibitors and Nefazodone. If given concomitantly with Zolvastin, these drugs increase the risk of Myopathy.
Fibric acid derivatives and Nicotinic Acid. These drugs further increase the risk of myopathy if given with Zolvastin.
Mibefradil. This drug is reported to increase concentration of Simvastatin (Zolvastin)
